The Office of Pupil Personnel Services, Special Education, and Central Registration assists the students and families of the Amityville Union Free School District in special education, ages 3-21; psychological and social work services; nursing and health services; and central registration.
A district Committee on Special Education and a Committee on Preschool Special Education are appointed by the Board of Education to review the individual educational requirements for all students in the district who have been diagnosed with disabilities that would interfere with their academic progress. These Committees are responsible for making recommendations to the Board of Education concerning an appropriate program or services, if needed, for each child with a disability. Parents/Guardians with questions concerning children with disabilities should contact the Pupil Personnel Services at (631) 565-6552.

If you are seeking guardianship, you must bring the following documents.
- Child’s Original Birth Certificate
- Photo ID of parent or guardian
- Physical Examination
- Current immunization records
- Guardian's Proof of Residency for owners or renters
- Our Guardianship packet (Fill out NYS Designation and Parent-Custodial Affidavit)
- Our registration packet, which can be found on the student registration page.
In keeping with the McKinney-Vento Act and other applicable New York State statutes on the homeless, a parent/guardian of a homeless child should contact Central Registration for assistance and due process rights at (631) 565-6552.
The following forms are to be completed by persons meeting the definition of homeless or unaccompanied youth or who believe they are homeless or unaccompanied youth.
PLEASE NOTE: Lack of immunization documentation for homeless students should be discussed with Central Registration. Homeless students are sometimes protected from exclusions stated in the immunization guidelines.
Special Education Programs and Services Focused Review
The New York State Education Department's Office of Special Education has completed a Special Education Programs and Services Focused Review in the Amityville Union Free School District during the 2017-18 school year. This review examined the district's compliance with selected federal and State laws and regulations governing the education of students with disabilities.
The Special Education Quality Assurance Long Island Regional Office conducted the review. The review process involved a review of school records of a representative sample of students with disabilities, classroom visitations, interviews of parents, and staff surveys. Meetings of the committee on special education were also observed.
