Office of Pupil Personnel Services, Special Education, and Central Registration
The Office of Pupil Personnel Services, Special Education, and Central Registration provides assistance to the students and families of the Amityville Union Free School District in the areas of special education, ages 3-21; psychological and social work services; nursing and health services; and central registration.
A district Committee on Special Education and a Committee on Preschool Special Education are appointed by the Board of Education for the purpose of reviewing the individual educational requirements for all students in the district who have been diagnosed as having disabilities that would interfere with their educational progress. These Committees have the responsibility of making recommendations to the Board of Education concerning an appropriate program or services, if needed, for each child with a disability. Parents/Guardians who have questions concerning children with disabilities should contact the Pupil Personnel Services (631) 565-6552.

Medicaid Compliance Plan
In keeping with the McKinney-Vento Act and other applicable New York State statutes on the homeless, a parent/guardian of a homeless child should contact Central Registration for assistance and due process rights at (631) 565-6552.
Homeless/Unaccompanied Youth Registration Form
(PDF) - This form is to be completed by persons meeting the definition
of homeless or unaccompanied youth or who believe they are
homeless or unaccompanied youth.
*PLEASE NOTE: Lack of immunization documentation for homeless students should be discussed with Central Registration. In some cases, homeless students are protected from exclusions stated in the immunization guidelines.
